About the Role:
Homegrown Essential Wellness in Smithtown, NY is looking for a passionate and dedicated Nurse Practitioner to join our growing team. This is an exciting opportunity to make a meaningful impact on patient health and wellness in a supportive, patient-centered environment.The patient consist of newborns, school age , adolescents and young adults. Responsibilities:
- Conduct comprehensive patient assessments, physical exams, and wellness evaluations
- Diagnose and treat acute and chronic conditions in alignment with evidence-based practice
- Develop and manage individualized patient care plans focused on overall wellness
- Prescribe medications, therapies, and lifestyle interventions as appropriate
- Collaborate with physicians and interdisciplinary care teams to ensure optimal patient outcomes
- Educate patients and families on preventive care, nutrition, and wellness strategies
- Maintain accurate and timely documentation in the electronic health record (EHR) system
Requirements:
- Active Nurse Practitioner (NP) license in the state of New York
- Master's degree or higher in Nursing (MSN, DNP) from an accredited program
- Current DEA certification and prescriptive authority in NY
- Board certification through AANP, ANCC, or equivalent credentialing body
- Experience in primary care, integrative medicine, or wellness-focused practice preferred
- Strong interpersonal and communication skills with a patient-first approach
- Ability to work independently and collaboratively in a fast-paced clinical setting
- Medicaid credentials a plus

Compensation: $58.00 - $70.00 per hour
